Anatomy and physiology of the stomach

It is approximately 6. Anatomically, the small bowel can be divided into three parts: the duodenum, jejunum, and ileum. In this article, we shall examine the anatomy of the small intestine — its structure, neurovascular supply, and clinical correlations. The most proximal portion of the small intestine is the duodenum. It runs from the pylorus of the stomach to the duodenojejunal junction.

D1 — Superior Spinal level L1. This area is most common site of duodenal ulceration. The descending portion curves inferiorly around the head of the pancreas. After the duodenum crosses the aorta, it ascends and curves anteriorly to join the jejunum at a sharp turn known as the duodenojejunal flexure. Located at the duodenojejunal junction is a slip of muscle called the suspensory muscle of the duodenum. Fig 2 — The different parts of the duodenum.

The liver, gall bladder and transverse colon have been removed. An ulcer in itself can be painful, but is not particularly troublesome and can be treated medically.

However, if the ulcer progresses to create a complete perforation through the bowel wall, this is a surgical emergency, and usually warrants immediate repair. A perforation may be complicated by:. The jejunum and ileum are the distal two parts of the small intestine. In contrast to the duodenum, they are intraperitoneal. They are attached to the posterior abdominal wall by mesentery a double layer of peritoneum.

The ileum ends at the ileocaecal junction. At this junction, the ileum invaginates into the cecum to form the ileocecal valve. The veins of the duodenum follow the major arteries and drain into the hepatic portal vein. Lymphatic drainage is to the pancreatoduodena l and superior mesenteric nodes.

The superior mesenteric artery arises from the aorta at the level of the L1 vertebrae, immediately inferior to the coeliac trunk. It moves in between layers of mesentery, splitting into approximately 20 branches. These branches anastomose to form loops, called arcades. From the arcades, long and straight arteries arise, called vasa recta. It unites with the splenic vein at the neck of the pancreas to form the hepatic portal vein. The ileocaecal valve represents the separation between the small and large intestine.

Its main function is to prevent the reflux of enteric fluid from the colon into the small intestine. It is also used as an landmark during colonoscopy, indicating that the limit of the colon has been reached and that a complete colonoscopy has been performed. The ileocaecal valve is also important in the setting of large bowel obstruction.

Should the ileocaecal valve be competent, a closed loop obstruction can occur and cause bowel perforation. Should the ileocaecal valve be incompetent i. Its name is derived from the Latin 'duodenum digitorum' , meaning twelve fingers length. Together these parts form a 'C' shape, that is around 25cm long, and which wraps around the head of the pancreas. D1 - Superior Spinal level L1. Small intestine

Small intestine , a long, narrow, folded or coiled tube extending from the stomach to the large intestine ; it is the region where most digestion and absorption of food takes place. It is about 6. A thin membranous material, the mesentery , supports and somewhat suspends the intestines. The mesentery contains areas of fat that help retain heat in the organs, as well as an extensive web of blood vessels. Nerves lead to the small intestine from two divisions of the autonomic nervous system: parasympathetic nerves initiate muscular contractions that move food along the tract peristalsis , and sympathetic nerves suppress intestinal movements. Three successive regions of the small intestine are customarily distinguished: duodenum , jejunum, and ileum. These regions form one continuous tube, and, although each area exhibits certain characteristic differences, there are no distinctly marked separations between them.

23.6 The Small and Large Intestines

The small intestine is made up of the duodenum , jejunum , and ileum. Together with the esophagus, large intestine, and the stomach, it forms the gastrointestinal tract. In living humans, the small intestine alone measures about 6 to 7 meters long. After death, this length can increase by up to half. It has a surface area of over meters. The internal walls of the small intestine are covered in finger-like tissue called villi. Each of these villi is covered in even smaller finger-like structures called microvilli.

The small intestine is the part of the gastrointestinal tract where much of the digestion and absorption of food takes place. The small intestine is the part of the gastrointestinal tract that follows the stomach, which is in turn followed by the large intestine. The small intestine is the site where almost all of the digestion and absorption of nutrients and minerals from food takes place. It can vary greatly, from as short as 4.

It extends from the pylorus of the stomach to the ileocecal junction and is subdivided into the duodenum , the jejunum , and ileum. With the exception of the proximal duodenum , which is supplied by the celiac trunk , the main arterial supply for the small intestine is provided by branches of the superior mesenteric artery. The veins of the small intestine drain into the portal vein and the lymphatics eventually drain into the superior mesenteric and celiac lymph nodes.

This article, part 4 in a 6-part series, describes the physiology and functions of the jejunum and ileum, the last two sections of the small intestine, where most absorption of nutrients takes place.
